What is CFSS?
Community First Services and Supports (CFSS) is a Minnesota healthcare program designed to help individuals remain independent in their homes and communities. CFSS provides support and assistance with daily living activities for children, adults, and seniors who need additional care due to disabilities, medical conditions, or aging-related challenges.
CFSS gives individuals greater flexibility, choice, and control over the services and supports they receive.
CFSS Services We Provide

Who Can Qualify for CFSS?
Individuals may qualify for CFSS if they:
- Live in the community or at home
- Require assistance with daily living activities
- Are enrolled in Minnesota Medical Assistance (MA) or eligible waiver programs
- Have completed a county or managed care assessment
- Choose the CFSS Agency Model
